For NZ Primary Care — Early Research Phase

Closing the loop
on patient care.

Smarter follow-up for safer care.

Built by a GP, for general practice

I'm a GP in Christchurch. My admin slots get taken up by the patients who need more time — and rightly so. That means on work evenings I'm back on the inbox: filing results, managing abnormal results, completing referrals, filing letters, working through tasks from nurses and other clinicians. General practice doesn't have much capacity left for the proactive, preventative care we'd all like to be doing. With GP shortages only growing, anything that gives primary care teams back clinical time matters. I couldn't find a tool that fixed this — so I'm building one. And I need your help to make sure I build the right thing.

Our nurses are brilliant. We're using them as text-message machines.

Every week, practice nurses work through recall lists — everyone due for cervical screening, blood tests, chronic disease reviews. They send texts, make calls, send a second text, then a third, then a letter. They check whether appointments have actually been completed. They follow up on results. They update and edit recall records. It's meticulous, important work — and it takes hours every week that could be spent in clinical conversations and preventative care.

Meanwhile, the capacity for proactive, preventative care gets squeezed out. Not because anyone doesn't care. Because there's simply no time left.

CareLoop is designed to change that. Automate the repetitive. Free the clinical time. Close every loop.

"I was looking for a tool to fix this and couldn't find one. So I thought — what if I just built it? Something affordable for small practices, with a real focus on equity and outcomes for Māori and Pacific patients."

📋

Automated Recalls

CareLoop manages the full recall cycle — generating lists, sending texts, making follow-up contacts, sending letters when needed, and tracking whether appointments have been completed. No manual nursing input required.

📥

Intelligent Result Triage

Context-aware triage that looks at the result, the trend, and the patient's full picture — then suggests a specific next step that is ready for your approval in one click.

↳ Mildly abnormal LFTs → book nurse liver function clinic + automated text invite ×3, outreach call if no appointment booked

↳ Mildly abnormal lipids + CVRA <5% → automated text invite ×3 to health coach lipids session

↳ Early-stage CKD → automated text invite ×3, outreach call, book CKD protocol nurse clinic

↳ PSA or TSH → intelligent suggested filing based on trend; always requires GP approval, even for normal results

🏥

Nurse & Allied Health Clinic Workflows

Based on results and clinical triggers, CareLoop routes patients to the right clinician — GP, NP, practice nurse, clinical pharmacist, HIP, or health coach — automatically and consistently.

🔔

Missed Appointment Detection

If a patient doesn't show, CareLoop detects it and triggers appropriate outreach — a text, a phone call, or a HIP contact for patients with mental health diagnoses.

💚

Proactive Care Outreach

Patients overdue for scripts, at risk of disenrollment, or with long-term conditions who've fallen off the radar — CareLoop surfaces them and initiates contact before they slip through.

🏡

Built for Small Practices

Affordable for independently owned GP practices. Designed to cost far less than the time it saves — and accessible without large admin teams or complex IT infrastructure.

CareLoop

Built to close the gap,
not just the loop.

Māori and Pacific peoples in Aotearoa experience a 6–7 year gap in life expectancy — driven by cardiovascular disease, diabetes, and late-detected cancer. CareLoop is designed so that automation actively works to address this — through care that reaches people where they are, not a system that waits for them to navigate it.

Prioritised recall outreach

Māori and Pacific patients with overdue CVRAs are flagged as higher priority — with calls before texts, not after. Because a system that treats everyone identically doesn't produce equitable outcomes.

Prescription access support

Patients overdue for scripts who are Māori or Pacific receive outreach that includes information about prescription subsidies and health navigator contact — not just a reminder text.

Whānau-centred contact

With explicit patient consent, CareLoop can flag a preferred whānau contact for outreach — recognising that whānau is the fundamental unit of hauora for many Māori patients.

Multilingual messaging

Automated texts sent in te reo Māori, Samoan, Tongan, and other Pacific languages alongside English — based on patient language preference recorded in their profile.

Equity reporting dashboard

Recall completion rates broken down by ethnicity — visible at practice level, so practices can see whether Māori and Pacific patients are receiving care at equitable rates. Most practices currently have no visibility on this gap.

Pre-disenrollment prioritisation

Māori and Pacific patients who haven't been seen in a long time are prioritised first in the proactive outreach queue — because disengagement from a system is not the same as not wanting care.
